All new patients are now asked to provide proof of identity as well as their NHS number on registration. Patients who wish to register a change of name are asked to provide supporting documentation and we always ask for a change of address or telephone number in writing to minimise the possibility of mistakes being made when taking a verbal message. Our patients may be asked to provide proof of identity when they register temporarily with another surgery whilst on holiday in the UK and it may be helpful to pack a slip of paper with your NHS number on it along with your sun block and beach towel. Please don’t be offended if you are asked for this information as we are doing our best to look after you.
